Assessments will  be conducted and provide a thorough evaluation to identify the clients strengths, weaknesses, and areas of needed improvement.

 

This providing critical information to support an accurate evaluation of the client and their needs, supporting accurate goal setting and evaluation. 

Reporting will be provided with education and feedback for clients and their support persons.
